Parent Permission Form:

Before conducting a physical exam, legal guardians must complete a parent permission form. This form is a standard requirement during student registration. If you have any questions or concerns about the form, please consult with the nurse. It’s essential to understand that this form also grants permission for the nurse to discuss health information with your child’s physician when medically necessary.
