Kindergarten Registration - Required Dental Form

Dear Parents,

We're excited to welcome your child to Kindergarten at Maroun Elementary!

Since your child is currently enrolled in the Phoenix Universal Pre-Kindergarten program (at either Maroun Elementary or the OCO Head Start site), they are already registered in the Phoenix Central School District.

To help us prepare for your child's successful transition to kindergarten, please complete the following steps:

1. We previously sent the Kindergarten Questionnaire. If you have not completed it yet, please do so and schedule a screening. This form will provide valuable information about your child's strengths and needs.
2. We are sending the Dental Form today. You will receive another email with the health form. Please download the forms, have your child's medical and dental providers complete them, and then upload them back to us.

Please submit all forms before scheduling your child's Kindergarten Screening, or bring them to the screening. Once the forms are submitted, you can schedule a screening by viewing the signup list and finding a convenient time by signing up on Parent Square or calling the main office at 695-1561.
